Where We Are
Gombe state located at the northeastern part of Nigeria and has an estimated projected population of 3,260,000 with a population growth rate of 2.4%. It has “the jewel in the savannah” as its state slogan.
The state has eleven (11) Local Government Area (LGA) divided into three (3) senatorial zones. Quality of Care (QoC) activities in Gombe started in 2017 but become more active in 2018. The program started with nine health facilities across the three senatorial districts in the state. Four Secondary health facilities and five primary health care are the initial learning sites. Each these health facilities have Quality Improvement (QI) team to put an eye on day-to-day activities of the facility at Facility Level with the same QI Team from the state level that influence the activities of LGA QI Team.
As of 2023, Gombe state with the government support and the development partners has scale up with additional 128 health facilities making a total of 137 QoC learning sites. In total, the state has all 24 secondary health facilities and 113 primary health care as learning sites.

Facts and Achievements
- The state is routinely tracking Reproductive, Maternal Newborn, Child, Adolescent, and Elderly Health plus Nutrition (RMNCAEH+N) related indicators using 30 QoC core set of indicators for quarterly report.
- To ensure continuous provision of quality services in the learning sites, the state made provisions for support staff such as the clinical mentors and coaches to support the facilities. At the communities, Community Health Influencers/Promoters (CHIP) agents are deployed. These agents are responsible for tracking pregnant women. They provide them counselling on the need to attend Ante natal and to ensure that their children complete their immunization schedule. This is a house-to-house activities in the communities.
- A score card has been developed for monitoring and supervision of services rendered by the learning sites. The scorecard consists of 11 QoC key performance Indicators.
- 11 QoC Indicators to track and rendered effective supervision of the learning sites.
