Where We Are
Kaduna State, situated in the Northwestern Zone of the country, and has an estimated projected population of 6,113,513, accompanied by a population growth rate of 8.8%. Recognized by the state slogan “the Centre of learning,” Kaduna is subdivided into twenty-three (23) Local Government Areas (LGAs), further categorized into three (3) senatorial zones.
In 2018, the Quality of Care (QoC) initiative was inaugurated in Kaduna State with the overarching goal of enhancing Reproductive, Maternal, Newborn, Child, Adolescent, and Elderly Health plus Nutrition (RMNCAEH+N) healthcare standards. A comprehensive baseline assessment was conducted, revealing that the 10 indicators stood at 50%, signifying the potential for improvement.
Since its inception, the initiative has extended its reach to cover nine healthcare facilities, thereby amplifying its impact on the state. Currently, 254 health facilities in the state receive quarterly funds disbursed as Direct Facility Financing through the Basic Health Care Provision Fund (BHCPF). This funding comprises Monthly Capitation from the Kaduna State Contributory Health Management Agency (KACHMA) and Quarterly Direct Facility Financing from the National Primary Health Care Development Agency (NPHCDA) Gateway.

Facts and Achievements
- In 2018, the Quality of Care (QoC) initiative was launched in the Kaduna State with the aim of improving RMNCAEH+N healthcare standards.
- The governance structure of QoC operates at the state level, with State Level overseeing the coordination of entities TWG in the State, LGA and Facility levels.
- QoC has received invaluable support from partners such as UNICEF, MNCH2, CHAI, PATHFINDER, CIHP, LSTM, IMPACT and BHCPF who share the vision of advancing quality care in the state.
- So far 254 Health facilities (in the states, out of the target 255 receive quarterly funds disbursed as Direct Facility Financing, through the BHCPF (Monthly Capitation from KACHMA and Quarterly DFF from NPHCDA Gateway)

To distill the essence of QoC’s impact into a concise format, a scorecard has been developed, focusing on three critical indicators: ANC coverage, Deliveries and Skill Birth Attendant (SBA)
- CHAI developed a set of QoC indicators for health facilities; and the data for these indicators acquired using three (3) different tools:
1. Observation tool: Direct observation of client-provider interactions and providers demonstration of selected clinical procedures, evaluating adherence to SOPs and guidelines.
2. Facility audit tool: To determine the readiness of each facility to provide FP/PPFP and PAC services.
3. Client exit interviews: To collect information about the client’s experience at a given health facility. Pulse oximeters, pneumonia, bio medical engineers.

- QA of HFs was conducted in the previous quarter to track progress across 10 priority areas, ensuring judicious use of the health facility funds. Quarterly QA of BHCPF facilities = only based line was done for 239 facilities in 2020
- The progress of QoC is closely monitored through the tracking of 10 different indicators, ensuring continuous improvement in healthcare outcomes.
- Since 2020, the Quality Improvement (QI) teams has played a vital role in implementing strategic goals and driving positive change within the QoC framework. QIT is enaugurated in the state 255 PHCs.

Significant increase in number of uterotonic administered within 1 minute of Delivery. Provision of Oxytocin and Magnesium Sulphate at the labour room for Afternoon and Night duty in all Health facilities.
Availability of screened Blood in the Labour room for emergency cases that need blood transfusion at Hajiya Gambo Sawaba General Hospital Kofan Gayan, General Hospital Kwoi and Yusuf Dantsoho Memorial Hospital Tudun Wada Kaduna.

Vaccines are made available in all shifts (afternoon and Night duty) in all the PHCs and Secondary Health Facilities.
Involvement of the communities in these learning sites, improved community awareness and facility deliveries.
Mama kits were made available in the 3 secondary health facilities in the state learning sites GH Kofan Gayan, General Hospital Kwoi and Yusuf Danstoho Memorial Hospital Tudun Wada for clients use and replacement.
On the job-training improved Peer learning and quality of care among the staff in all learning sites.
